How to Access Treatment - For a Friend or Relative
If your friend or relative is working with a community treatment agency, they should discuss their preference with their care coordinator who will make a referral on their behalf.
Those in a position to self-fund treatment should contact our admissions department on 01934 815515 to discuss treatment options and requirements. Our staff will take some details and contact your friend or relative directly to further discuss our treatment programmes. Should they wish to proceed towards admission we will request that they contact their GP to give permission for them to release to us, relevant information including:
· Recent blood test results where necessary
· A patient summary report
We will review the application and contact them with details of a time and date to conduct an assessment. The assessment will cover any physical, psychological and emotional health needs. We need to ensure that our facilities and programme are suitable and meet their needs. Since the aim is rehabilitation, patients must be able to self care and have sufficient cognitive function to understand and engage in the programme.
Once our assessment has been completed and both parties feel that the treatment programme is suitable, an admission date will be offered.
